• At home assessment with recommendations – monthly to weekly visits
• Collaboration with medical team
• Written reports shared with approved individuals or medical team
• Home safety evaluation
• Dementia-specific educational coaching sessions with family or paid caregivers
• Meaningful activity recommendations
• Facilitation of family conference calls
• Coping strategies recommended along with emotional support
• Assistance with transitioning to long-term care
• Follow-up evaluation and reports within assisted living, memory or skilled care
• Phone consultations
• End of life planning
• Coordination with palliative and hospice care
